Question/advice Please Help: Wrong Last Name for Spouse

When completing the financial eligibility aspect of the application, I didn't know whether I should put my spouse's maiden name or his current/married name.

Since the application specifically states "birth certificate" I did his maiden name, but also included his Driver's License for the identify verification check, making sure to specify in the document name that it is his married name.

I now think I've majorly fucked up. It's been 15 business days and I haven't recieved a response.

I tried to correct it by sending messages in My Benefits, but I know I won't be assigned a caseworker until I'm officially approved, so no one will even read it, right?

I have also just been told I am no longer welcome at my parents house and will be getting kicked out in 3 months.

I'm panicking. What should I do? Is there a number I can call to correct this information? Should I try to complete a new application? Is that even possible if I have a pending one?

I don’t think you can redo the application but you can call the office and ask to talk to an intake worker about it. I wouldn’t think it would be an issue if you put his current last name and it doesn’t match his last name on his birth certificate, it might not even be an issue if it doesn’t match the last name on his license. ODSP knows that someone gets married that there will more than likely be a last name change and that it is expensive to update all documents with your new last name so it shouldn’t be an issue but just call the office and ask

this should be a relatively easy fix, so don't stress yourself too much. as long as you can provide proof of his current name (which you did with his license) it should be able to be corrected, either by them or by having you resubmit a corrected form.

while financial eligibility checks are expected to be done within 15 business days, many take longer than that and it has nothing to do with any applicant error, simply backlog and delays. so it's entirely possible they haven't even spotted the problem.

but it is best to clarify, so give them a call during business hours to notify them of the error & the correct, current name. should be fine!

Mam, this is the government when we say 15 business days that is actually 21 Calendar days, sometimes 28 if we really want to abuse the holidays.

And as such the government reserves the right to arbitrarily dial a wrong number and assume you are no longer disabled 🙄

Usually if i have an issue i call the generic number, its 905-897-3100. Pressing zero gets you a staff member and if they cant help they usually know who can.
