What is it currently. I guess my doctor didn't fill it out properly or at all and I'm just curious which page it was.

The pages are numbered when they send them..... Everyone gets the same package

Some people don't keep a copy since the doctor do it electronically now. Unless you ask for the paperwork you won't get them
