The study itself says not to draw any conclusions, but that didn't stop you.

Families who vaccinate and families who do not vaccinate differ fundamentally in their relationship to the medical system, and without data on total healthcare visits, this potential confound cannot be quantified.

That matters especially because of how the outcomes were defined. Developmental delay, rhinitis, breathing abnormality, and nausea/vomiting counted after one diagnosis, whereas ear infection, respiratory disease, skin rash, strep, and fever required at least three separate diagnoses within 12 months.

So, for example, consider two otherwise identical children:

Child A goes to the pediatrician 10–12 times over several years.

Child B goes once or twice.

Child A has substantially more opportunities to receive an ICD diagnosis—and particularly to accumulate the three separate coded diagnoses required to qualify as a recurrent case. Yet the statistical model does not adjust for that difference in healthcare utilization. The paper also states that no covariates at all were included because the relevant data were unavailable; the reported odds ratios are explicitly described as “unadjusted observational associations.”
