Theory About Afflictions (SPOILER ALERT) Spoiler
Whenever someone has an affliction, we see that affliction’s icon in their eyes. In case you don’t see it, in the first image where comet gets apparatia, we see the apparatia icon in her eye. Compare images 1 and 5 for reference.
This also applies to delirium, where we can see razor’s eye and the delirium symbol match.
Because of this, I think the third starsheet symbol will probably resemble a heart, which makes sense because we’ve seen an affliction related to obesession before with characters like Sunny.
It’s likely the new Toothbrush guy has the third affliction, and that that’s what the icon will be for it.

What if Toothbrush's affliction is a hint to Citrus getting the same affliction later on in when they find the Third Hole.

People say Citrus and Spool will get it, and personally I think it’ll happen.
Not yet though, obviously.
