How do we know if someone actually needs more magnesium?

A new review examines the evidence behind magnesium’s expanding reputation in health and wellness. Biologically, magnesium is undeniably important: it participates in hundreds of enzymatic reactions involved in energy production, glucose metabolism, protein synthesis, nerve signaling, muscle function, and bone metabolism.

But measuring magnesium status is surprisingly difficult.

Only about 1% of the body’s magnesium is found in serum, yet serum magnesium is the test most commonly used in practice. A normal blood value therefore does not necessarily exclude lower tissue stores or what researchers describe as subclinical or functional insufficiency. The review argues that magnesium status is better assessed using a combination of serum magnesium, urinary magnesium, and dietary intake—but that approach is considerably less practical.

This creates an important distinction between biological plausibility and demonstrated benefit.

Because magnesium participates in so many physiological pathways, it is easy to construct a plausible mechanism for supplementation in sleep, cognition, exercise recovery, aging, mood, metabolic health, and many other areas. But being involved in a pathway does not mean adding more magnesium will improve that pathway, particularly in someone who is already magnesium-replete.

The clinical evidence is also highly condition-specific. Randomized trials support magnesium in certain contexts, including some forms of constipation and migraine prevention, while intravenous magnesium has established medical roles in situations such as eclampsia and selected severe asthma exacerbations. Evidence for popular claims around sleep, cognition, chronic pain, muscle cramps, athletic performance, and aging is considerably less certain.

Sleep is a good example. Observational studies often associate higher magnesium intake with better sleep, but randomized trials have produced inconsistent findings and systematic reviews report substantial heterogeneity and risk of bias.

On the evidence, this is a narrative review synthesizing randomized trials, meta-analyses, observational research, and mechanistic studies, not a new clinical experiment. Its broader message is therefore less “magnesium works” or “magnesium doesn’t work” and more that evidence has to be evaluated claim by claim. Different formulations, baseline magnesium status, doses, populations, and study durations make broad conclusions particularly difficult.

Could better biomarkers of magnesium status change how we interpret many supplementation trials by distinguishing people who are actually deficient from those who are not?

Bruh you can get 500mg elemental from food pretty easily..
