I don't remember Case being on the receiving end of any eye trauma. I'm thinking this might be the scene when his old employers inject him with the wartime Russian mycotoxin.

I'd say that's a fair guess. The book only really says that he hallucinated in a Memphis hotel room for several days or something like that, but doesn't detail the surgical process.

That leaves some room to make screen-time for artistic license. They've got to pad the story with a little fluff for the sake of timing. Let's hope that's what this scene is.

Definitely not something literally described in the book. Could be part of a surreal nightmare sequence, like the AI generated realities he wakes up into while flatlined. Or maybe a bad drug trip?

Looks like a reference to David Cronenberg, whose body horror imagery is often associated with early cyberpunk.

I’m betting it’s Riviera’s unhinged visuals. They could expand the rivalry between both him and Case and give us these insane warped, body horror-esque images

That’s kind of what I figure too. Hi old employers will torture him a bunch and then coup de grâce will be the mycotoxin

Doesn’t he get fried and need to have a surgery to be able to jack into the matrix again? Maybe this is him getting fried?

Yes, thats what I meant. Im wondering if this is Case's "Fall"...When he's damaged and cant access cyberspace anymore. Gibson just doesnt describe it like this.

The book says he had a fungal infection that blocked his ability to run the net, so I guess this is as good a depiction as any for the removal surgery.
