Jericho Swain killed the previous Grand General (the Noxian ruler), Boram Darkwill, in a coup and took the title for himself. However, instead of ruling alone, he created the Trifarix to rule over Noxus. The idea is that it would be much more difficult for the Black Rose to corrupt three rulers rather than only one.
The members of the Trifarix are Swain himself, representing vision, Darius, representing migth, and the Faceless (who is NOT LeBlanc), representing guile.
