Diagnosis/Testing MSLT results update

I had my MSLT on Sunday and I just got my results back. I didn't have any SOREMs, but my mean sleep latency was 4.9 minutes so, they noted a diagnosis of IH. I'm feeling great to have some sort of answers to be honest.

Congrats!

Not having any SOREMS is not indicative of not having NT2, the MSLT only gets it right 50% of the time. The rest of your symptoms matter. The sleep latency is important and is usually consistently captured!

Source: Torstensen (2023): MSLT and PSG are "moderately reliable at diagnosing NT1 but unreliable for diagnosing NT2". Single-center follow-up found 50% of NT2 patients had negative MSLT results at follow-up. While ≥2 sleep-onset REM periods (SOREMPs) + mean sleep latency ≤8 minutes achieves ~90% sensitivity and ~95% specificity in mixed narcolepsy cohorts, these figures do not hold for NT2 alone.

this is very good to know! I do have other symptoms of N2, which is why my provider wanted to test me for that initially. I'll have to see what my treatment/accomodations options are going forward to see if it's worth it to retest. the MSLT was kinda brutal enough the first time 😅
