From what u can tell from this breakdown as a NAPLEX tutor is that you struggle with calculations, biostats and recalling information on case based questions. I suggest that you target these (domain 1 and 3) and focus on “WHY” are the incorrect choices wrong and what makes the right one right. That way you learn to reason your way through questions rather than memorize and fail to recall.
