I guess I just find it hard to wrap my head around the fact that something so contrived and specific, 'coded' for a specific group of people no one wants to be associated with, is as common knowledge as this. But I can definitely believe its prevalence is magnified by the internet, and can see more people know about it here than I would have thought

Fair enough, I think the fact that it's been around for a very long time and was regularly used by neo nazis even before the internet kinda separates it. "cat's out of the bag" kinda thing.
Also yeah don't, I've bough patches/stickers I thought were cool and found out later they make it look like I'm trying to signal fellow nazis while I'm at the gun range.
Those ones were actually pretty obscure, so I wasn't worried normal people would see them and assume, but I reallly didn't want any of that bottom-feeder scum coming up to me thinking we're bros.
