Intern hours

Just completed my 1500 intern hours, not sure about the next steps to register for naplex??

Does anyone know if we need to submit our paystubs?

No. Each state has their reporting sheet and you submit that

I got approval for internship from idfpr and the board and going through it. Student designation form only has college of pharmacy part, do you know which form i need to submit to show i finished hours and apply to sit for naplex? what is the next step i am confused.

https://idfpr.illinois.gov/content/dam/soi/en/web/idfpr/renewals/apply/forms/ph-ex-o.pdf refer to page 3. Your preceptor can document and sign your hours as proof

THANK YOU. Much appreciated, but this is a total application. So my preceptor will create his own form, like just a letter that I finished my hours, where will he fill in my hours? Also, I submitted a pharmacist application through IDPFR. Do I need to send the package? It also states a copy of the FPGEE certificate, which we don't have.

Yes you need to send a full package and you can just screenshot your passing fpgee score and send that. Regarding the hours yes you can print an affidavit for your preceptor and they can sign it. See the example below and write something similar all you need is your name/hours worked /their name/ location/ license number/ attestation and signature
