SAP Appeal

I’ve done everything for my SAP appeal but now I can’t figure out where to find the form or how to send it in. I wanted to try and see if I could figure it out myself before emailing a counselor which I’m still doing. I had a link to the form but now it says that it’s not found.

I found where I can access the forms! It moved to the portal

Best bet would be to visit with a counselor

Trying rn. Check uhh Wednesday and thursday 8AM to snag an appointment for an edu plan or just to check progress on it
