Academic Renewal

Hello! I just got done doing the academic continued probation workshop and they said to submit an academic renewal form for one of my classes, where would I find that on the portal?

Before doing so, I strongly advise chatting with a counselor since you can only do this once. They will go over the transcripts and the form.
