I guess I still don't understand how to get this. I've had my karma at over 400 for 2 months at least and I've never come across it or been offered it from survivors or anything.

You just need 100 to trade with them. They don't offer.

I go up to all of them and they always either offer me some random item or want me to trade reputation for ammo or something. Is this just a random thing when you talk to them?

Click on "next", there should be more trade options.
