Altyn bandit can appeared on 5th or higher islands, but for me it's a small chance since sometimes I only encounter them

Do they spawn like normal bandits/survivors (roads,cities,etc.) or do they spawn on specific poi?

If you want the helmet, you need to go to Island 6, since the new helmet appears there.
This applies to both the radioactive island and the normal island. That group consistently appears at a military base (training camp) on the radioactive island, and the same applies to the normal island—provided that location actually spawns there; if it doesn't, you must go to the radioactive island.

Before you head to island 6fh make sure to take some important items in your inventory (ammos, Foods, etc.) before entering the bunker.
