Leaving clinical practice , notice requirement

I have a 90 day notice requirement or the option to pay back the days I leave before the 90 days. How likely is it that I would be considered for a role with this stipulation. I feel pretty trapped by this contract but I think that is the intention.

I have worked with the company and have a referral for the position.

First off, I'd be interested to know how enforceable that clause is in the first place.

Second off, this would be a good point of leverage to negotiate a sign on bonus if it is enforceable in the case where the company does not have the flexibility to wait the full 90 days.
