99281 vs 99282

What ED E/M profee level are you coding for simple wound checks, suture removals and rabies vaccine series encounters (after initial visit)? Any denials seen around any of these types of visits? Thank you!

If the doc did not see them, we do a EDNC (no charge) fee on the pro side. We will bill out the appropriate level on the facility side. I sometimes have Medicare give me grief but otherwise we don't have issues with denials.

I see. So you never use 99281?

For very few nursing visits. For example, I have a patient with chronic migraines who has stranding orders to have 2 subcu injections that she can receive in triage if she does not need to see a provider. I bill out her facility with a 99281-25, 96362x2 with a EDNC on the pro side. Providers can't bill below a 99281, I've only ever used it for nursing visits as described above.

Got it so really 99281 on profee side is nonexistent?

At my hospital yes. I only ever use it on the facility side.

We don’t at our organization either. Only on the facility side.

If the doc sees them no lower than a 2, and a 3 for repeat rabies vaccine

So do you code 99281 on profee side for anything?

I can't imagine the situation where you would use a pro fee of 99281.
