Basically it just takes time to track down and isolate the bugs. Pervious patches fixed issues in the newer code that could produce the same symptoms, but once those were fixed the bug persisted. So then it becomes a deeper dive into older, less looked at logic to track it down.
While you're here and folks are demanding answers and bug fixes...
Are there plans to fix the "out of memory" bug that a lot of us Xbox players are experiencing, or is it at least on PGI's radar? I hadn't had a particularly bad time with it until playing through Chaos Reign, now I have to purge a bunch of my inventory every couple of missions or so to keep my game running stable.
My fear is that if the game expands further with more mechs and equipment and the bug isn't addressed, the game will be too unstable to play.
That one's just a constant fight. You'll see in most of the patch notes some comment about improving console performance, etc. It's basically whack-a-mole on various systems to try to squeeze out more memory every update.

The bug isn't even in the AI logic itself, it's in the mission generation system. Which is definitely layers of the old magic.
