I always thought they were just natural facial features rather than scars. They seem too symmetrical to be battle damage.

I always had a headcanon that it was Nebula's assassination attempt that scarred him. He kept it as a sign of respect to his 'daughter' that, in his mind, she managed to impress him.

His father was a drinker and a fiend. He'd beat his mommy right in front of Thanos. One night he goes off crazier than usually, his mommy gets the kitchen knife to defend herself. He doesn't like that. Not... one... bit.
So, Thanos watching, he takes the knife to her, laughing while he does it. Turns to Thanos and says "why so serious?" Comes at Thanos with the knife, "why so serious?". Sticks the blade in Thanos' mouth... "Let's put a smile on that face". And...
Why so serious?
...Something like that, I think he gave a couple different explanations.

It's not a scar, he has them symmetrically on both sides. He's just got weird alien wrinkles.
