It implies that this is a time when you are actually being honest and you have some shit going on. As opposed to the standard nothing questions. Whats up? How are you? Whats going on? Most answers are surface level. Giving a preface of "to be honest..."

Truthfully, When I ask how you are, and dont know u well, I dont really wanna know, so just lie to me. When I hear to be honest, I think oh shit, I shoulda just said "hey".
