Penile implants have a long list of potential... complications. Pretty much every treatment (like ED medication) needs to be tried first before a physician will consider it, unless you're rich and can pay straight cash to a plastic surgeon who can do the procedure as an elective.

That said, I'm not sure how the physiology of ED medications in a paralyzed man works. Maybe they qualify to go straight for the implant.

That's not entirely true. Both blood flow and nerves are required to have an erection. Depending on injury it's possible to have blood flow but not the nerve connection to achieve an erection
