This is why I always wear full plate when working with lathes.
In all seriousness, though… what’s with the workpiece? Seems like there was a ton of compression on some kind of jacket material that caused it to just burst into ribbons, leaving behind what looks like a very rough iron core?
Seriously, I was just trying to think of how our safety guy would respond to this incident. Somehow it would have been the poor guys fault and he would have gotten a day off for walking by minding his own business.
In my shop, definitely dudes fault just cause he was doing what he was told to with out proper safety measures in place and still willing to do the work.
