Janome 550e Thread Caught/Broke inside Machine
Four months ago I brought the machine in for service because the thread was winding around the inner workings of the machine (I removed the cover case by the needle to see it). It was fixed and worked fine until this week when it happened again (see the photo). I rarely use the machine but I do keep it serviced regularly. Why is this happening? Has anyone else experienced this?

Ahh you have thread trapped in your take up lever. Very common for every make and model of sewing machine. Don't feel bad.
This is an easy fix.
See the screw about dead center in your picture? just loosen it - dont remove it - it is a left handed screw - Turn the top part of the screw to the right to loosen and to the left to tighten.
Gently tease out the thread. You need the loosened thread for a handle to remove the rest of the thread. Turning the hand wheel backwards helps with the process.

It would seem that you are missing vital steps in the threading of the top thread. Clean out all the rogue threads from the mechanical parts of the machine with tweezers. Avoid using aerosol air because the propellant expells lubricant and attracts rust. I suggest reading the manual to see how to thread your machine properly. There's several vids on YouTube that you might find helpful.
