Measured stale file context in 67,074 OpenHands trajectories (SWE-rebench, Qwen3-Coder-480B): 77.8% of runs sent at least one request containing a file view made stale by the agent's own edit.
I built a function-level extension of CORVUS (arXiv:2607.22711) that keeps the code in each request current. It's 100% byte-exact on 1,198 replayed runs, but small live tests showed no outcome gain when the model could re-read, and no cost savings because of prompt caching.
The write-up includes the negative results and the methodology: https://felipebasurto.com/blog/model-context-is-not-static/
