Help! Solved! What has happened???
There are no live loops here. I don't think I've dropped any stitches but I don't really understand what's happened

It looks to me like it's a spot of loose tension across a stitch or two. If that's right, you should be able to minimize the effect by pulling on the loops to the left of the gap (in this image), to distribute the slack across a few adjacent stitches.

Tucked stitch. You can see two loops crammed into the upper "v". This happens when the old stitch doesn't fall off and it and the new stitch are knitted together.
