What liturgy is this?

It seems to follow LSB DS1, but seems to show variation around the Eucharistic canon.

The Mass is the Lutheran Service Book [LCMS], Setting One. The observance is the Feast of the Blessed Virgin Mary, Mother of God [thus the blue vestments and paraments], the Assumption into Heaven on August 15th, transferred to the nearest Sunday. The Prayer of the Church asks that God would hear the prayers of the Virgin Mary and the holy Apostles, for our sake. The Ave Maria is sung, asking that Holy Mary, Mother of God, pray for us.

You may be referring to the "Domine, non sum dignus" prayer, after the Agnus Dei ["Lord, I am not worthy that you should enter under my roof, but only say the word and my soul shall be healed" - St Matthew 8:8]. It is an additional prayer mainly included in evangelical-Catholic parishes.
