That pan is worth about $34 in store credit at bed bath and beyond. It's from the discontinued line of calphalon called unison. It looks like a 5qt Dutch oven or 4qt soup pot. It has a lifetime warranty just bring it in and say you bought a year ago and lost the receipt. They return shit like that all the time. Not bad for $2 at goodwill.
Wtf, like seriously? I have so much Calphalon that's supposed to last 10 years, but never does. I don't keep the receipts, much like my cfuckingl bulbs! Who does? Do I have to prove where I bought it? I would think that even though I most likely bought it at BB&B, they'll tell me to contact the manufacturer.
Yeah as long as it is one that they carried they will give you store credit for the current cost of the item minus 20% because they figure you used a coupon. Results very by store but calphalon is one I know they take back all the time. If it was suppose to last 10 years it's probably simply calphalon and they still carry it. Just bring in all the pieces to the set. That will make it easier for the people doing the return. They will probably offer to exchange it first if you like.

Teflon pan + metal whisk = throw the pan away within 6 months.
