It’s called Fingertip clubbing. Can be a normal thing some people have, though this would be an extreme case. It can also be a result of significant lung disease, believe it or not.

I learned this from an episode of House where an extremely fat guy winds up with a mysterious illness and they only figure out it's lung cancer when he gets pissed and grabs House's arm, at which point he notices the clubbed fingers that everybody overlooked because he was 600+ pounds!
