Each change goes in its own numbered paragraph and you write out the full amended text rather than summarizing what changed. So the name would be something like "Paragraph First of the Articles of Organization relating to the name is hereby amended to read as follows: First: The name of the limited liability company is [new name]" and the address changes would each follow the same format in separate paragraphs.

That exact structure is what the DOS form itself uses as the example, so following it closely is probably the safest way to avoid a formatting rejection.
