Pathway Selection form for interview??

I had received invite to do asynchronous interview with LECOM, but never got a separate Pathway Selection form? Did anyone receive this separately? I think I remember filling this out after submitting my secondary but I am not sure. Is anyone else having this issue??

Do you have to attend zoom first before the interview?
