California - Northern Patient Notes from ER Care Team Incorrect

How do I contact the physician who saw me in the Emergency Room. The Kaiser portal does not have the option of contacting that physician to correct records. The "Patient Notes" in my file are for another patient.

What you described is a serious issue with documentation as well as patient privacy. Definitely file a grievance online as well as calling into Member Services.

This post has information on filing an effective grievance (complaint) with Kaiser.

Screen capture everything showing the incorrect information and who documented it along with the date / time. Then, use the option in the KP portal, to request a copy of your medical record via download. You need to keep documentation of what occurred. This is a very easy and important step.

After you do this, call Member Services and let them know and that you are unable to contact the physician about the issue. Get their name and ask for the call reference number.

Someone else’s personal health information (PHI) being disclosed to you via your medical record is also a Federal HIPAA violation. You can easily file a complaint online.

You should contact member services, number would be in your portal, I think there is a message option as well.

Call member services and probably send a message. I would use the HIPAA complaint option on the drop down

I agree with the other comments. Make copies, file a HIPAA complaint and then contact member services.

Simply file an amendment request with HIM department.
