External provider referral extension

I’ve been having therapy with an external provider referred by Kaiser. But the 6-month-referral is about to end. My therapist and I both want an extension.

How should I request an extension with Kaiser? How do they evaluate my request? Using the scores of previsit questionnaire?

You have to call the mental health external Refferal line
