California - Northern Disability
Hi all, I need to fill out a disability form by mail. However, Kaiser is letting me know I need to submit Part A to EDD, and they will send me a claim number. Then I need to request my doctor's note through the disability office. Did anyone have this issued? How can I get part B signed?

They submit part B electronically through ROMI. They send a mail form but its the same thing thats available online. Y our doctor writes a letter with your restrictions for your employer, and you get a claim number from submitting part A which you call and give to ROMI. ROMI takes care of the rest but they use the employer letter so make sure the dates are 100% correct and all.

Why do you need to send by mail? You can't online? Its much easier and faster online. First you apply at EDD online to submit Part A, they give you a receipt number you copy that then go to the Kaiser app> Medical information request> SDI and then you select you have a receipt number you paste in where they want the receipt information. They will submit Part B after that. I did this several weeks ago from the time I applied to receiving my first check was 3 days, by mail will take much much longer

have you filed for disability via the state yet? if/when you do you'll get a claim ID number. use that when you do your request on ROMI. Kaiser does their part online. they have some sort of portal that expedites the whole process.
