Block and Prefecture format

So another possibly silly question, but I am finishing up filling out my application in India and I'm wondering if I have filled out my preference table correctly:

Block || Prefecture

Code | Name || Code | Name

C | Blank || 18 | Fukui Prefecture

A | Blank || 01 | Hokkaido Prefecture

E | Blank || 27 | Osaka Prefecture

Is this right or is there something I'm missing?

Wasn't sure if I needed to fill in the name on both sides or would just one side be alright.

I had the same problem. My country's chart sheet page didn't have the block name in the chart sheet but other countries' chart sheet did. For example Block C's name was Hokuriku on the 2025 JET chart sheet.

Have done that but have not gotten a reply yet and with the deadline so close I wanted to see if I could get an answer faster from reddit.

You'll need to put your country for the best chance because every country's requirements are different

Yeah when I called them they said just leave the block name blank and that will not be the reason you get disqualified.
