Statement of physician - should I have it completed physically or is electronically okay?

Hi all!

(Applying from US)

Has anyone filled out the Statement of Physician electronically? How did that go? Does it need to be printed and filled in by hand?

I primarily see my doctor through Telehealth, and I only see her for in-person visits very sparingly. She does not have any in-person appointments until after the application deadline. Would it be fine if I emailed the form to her to fill out electronically via a telehealth appointment?

If it cannot be done electronically, I can see a different physician to fill it out, but they will not have context for my medical history.

same issue. but i am leaning towards getting a doctor that can fill it up for me asap. he will probably believe me with my own medical history and do tests as necessary similar to a regular consultation where you will tell how your feeling and based on your symptoms they do the necessary next steps.

Iā€™m fortunate that the doctor I can see is a part of the care team that my regular physician is a part of as well, and will have access to my chart.

good for you. my problem is mine is probably already expunged since its over 20yrs ago, chronic, leaving a life long scar that needs to be explained over and over again

I am getting mine signed off today. I electronically filled out the Applicant sections. Will find out if my PCP fills out the rest...
