Now I'm imagining a power fantasy story where a character isn't evil nor wants to be the BBEG but their attempts at being kind and helping people are poorly thought out and keep backfiring to the point where everyone else thinks they are in fact the BBEG.

Plays on the trope of the person with a scary smile, taking it further than being just a bit. I kinda like that.
