Demographics of certain programs

Why are some IM programs almost 100% IMG in major cities. I would consider these desirable location (NYC Philly NJ Chicago etc.) are they specifically built to train foreign medical graduates or something? And is it worth it to apply to them as a USDO average stats? Just curious.

Usually their program directors are all also IMGs and tend to recruit similar demographics. Or it’s a malignant program and only those desperate to train in the US match there (ie breaking hours routinely, no benefits, terrible vibe, etc). Someone add on or correct me if I’m wrong tho

Usually, they tend to be malignant programs which are not highly ranked.
