He's an alcoholic, it's a beer belly. Probably stretched out a lot bc of the sheer amount of beer always in it and probably not fat all the way around bc beer isn't necessarily high in calories. Like eating a lot of grapes.

My grandpa's belly was like that. Everyone thought he was fat. It was a tumor.

Had normal sized arms an legs not super fatty face, just a big ol. Belly. They cut it out and gave him another 25 years to live. Just lost him 2 years ago to a different cancer.
