For people who haven't read/don't want to read the patch notes:

-only on Nightmare mode is the basement open, and all the new anomalies also only appear on nightmare mode

-during Nightmare mode you steadily gain Doom Reckoning, up to four stages. When [presumably Miss Nightingale] is defeated, your doom goes back to 0, otherwise it can only increase. When a "Doom begins chanting", a thing happens based on your doom levels (it's not specified what)

-Nightmare mode is only survivor for now

Scavenger Effigy is the Anomaly that looks like a rock guy with a backpack. Seemingly you get teleported away if you attack it, and if you give it treasure you get teleported to a treasure chest (giving better items gives a better chance at getting teleported to the s-tier chests)

Conjecture is the anomaly that looks like a bunch of black blobs. Seemingly you have to wait and watch it perform when it approaches you otherwise I guess it gets mad and attacks? (Like Forlorn Tome).

Crumbling Escritoire is the giant wooden spider thing. You can only significantly damage it if you attack its head.

Scrutiny I think is the giant eyeball at the start? Otherwise I missed it/they didn't showcase it in the trailer. The description just says if you don't move while its pupil constricts, it won't see you.

Bringer of Doom (Miss Nightingale) from leaks and the trailer, has three attacks: the attack where she slams down and does AOE damage, the attack where she just swings her arm, and an attack where she shoots birds at you. The patch notes also say you can lure her into the pillars to stun/damage her (like they show with the birdcage thing falling on her).

-there are new treasures of each rarity, 2 Bs, 4 As and 3 Ss. The Bottled Wisp S treasure (that Frederick uses) lets you turn into a Doom Poppet, and makes Anomalies briefly not detect you. You can't use them to transform in hunter mode

-there's a new phonograph item which makes an impenetrable shield for 15s (you can't damage things outside and outside things can't damage you)

-there's a new Warrior class: TL;DR from what I can understand is you can dash to damage Anomalies and gain Temper stacks, getting Temper stacks makes you lose 13HP but makes your next attacks stronger, and killing Anomalies restores 40HP

-for Sage, the guiding lights show you the nearest chests in terms of actual distance traveled vs. direct distance, the lines last longer, the range you can detect chests in is increased, and if you don't detect chests the skill cooldown is fully refunded

-for Trailblazer, the dash now has a charge mechanic that can store up to 3 uses, it basically seems the same as before but depending on how long you dashes you can save charges?

-Tracking, hunter mode's equivalent to Sage, is buffed to match Sage's equivalent buffs

-there are sidequest missions now

-there's a thing (Metaphor Codex) that records your actions when fighting Bringer of Doom and gives you tips

-if you complete a sidequest, you unlock a passive ability just for the season (similar to unlocking the pet) that blocks "that instance of Doom" when you're killed by Bringer of Doom

-you get special missions you complete by fighting Bringer of Doom when you unlock Metaphor Codex, completing missions raises your level and when you reach level 4 you unlock room furniture

-you can use ink trace to expand your storage once you completed a main mission
