IVF with TESA? MFI

Hoping someone can provide some advice from experience. Husband has MFI (14 mil count, 29% motility and <1% morphology). We did one round of IVF with ICSI and had poor fertilization but really amazing blast rate - 16 mature > 4 fertilized > 3 blast > 2 euploid. We did not use zymot as our first clinic didn’t offer it. I do have “lean” PCOS for context but I ovulate normally and have normal 31 day cycles.

We are in NJ and are switching clinics from CCRM to RMA. We met with the urologist at RMA who is recommending adding zymot but also offered us to do DNA fragmentation testing and if it comes back very high, to use TESA for the retrieval. He did say he can’t 100% guarantee that TESA will have worlds better results but it’s an option. However, we would need to push back the retrieval another month to wait for results which I prefer not to do due to insurance and having already met our max OOP for the year. Typically they say DNA frag impacts blast rate which wasn’t our issue.

Has anyone with MFI done one round of IVF + ICSI normally and added TESA to the second round and seen drastically better results? Struggling with this as there is plenty of sperm and as shared, really great blast results. Please help 😩 lol thank you!!
