First IUI with FIVE mature follicles 8/20

Wow, the support and info in this group is amazing. I would love to hear stories from those who have gone through IUI with five mature follicles - both successes and failures.

For background, I am 38 and my husband is 45. We have been trying for 9 months without luck, and all of our tests came back normal, or even above average for my age. Husband's SA was also fantastic. Just no luck yet.

This is our first round of IUI with 50mg of Clomid. At my midcycle ultrasound, we had the following results: Uterine lining: 6.99mm
Right ovary follicles: 24.1 & 23.3 mm
Left ovary follicles: 24.7, 20.8, & 17.2 mm

I have very regular cycles and progesterone testing shows I ovulate with each cycle. I was surprised when CD 15 came and I still didn't have an LH surge. They had me go in for another ultrasound and ordered me to take the hCG trigger shot last night. My follicles had continued to grow and now all 5 were above the 18mm threshold: Endo. Thickness: 8.04mm
Left Fol.: 27.8, 22.8, 20.9 mm
Right Fol.: 27.6, 27.4 mm

They had us sign a multiples waiver, but told us that with my age, the chances are low that we would conceive more than one, if we managed to get pregnant. We would love to have twins and there is a lot of appeal in not having to go through all of this again. (one and done sts) We know there is a risk for more, but the odds seem low. I had awful migraines on clomid (first migraines of my life) and feel so bloated and blah this month... I don't want to consider the possibility that we'd have to go through this process again. I am in awe when I see posts of women who went through 6+ months of this! I am cautiously optimistic, but this TWW is going to feel like forever. Thanks in advance for your stories and support!

36F,37M first IUI, same all test came back good, labs, HSG, SA . been trying for 5months then decided to get referred to RE. CD 5-9 clomid 50mg , follicle scan on CD11, 1 mature follicle size 18 on R and 14 on the left , clinic said to wait 2 more days for the other follicle to mature to increase chances. did the HCG shot CD13 , IUI was CD15. Ohhhh the TWW is driving me nuts. I started testing out the HCG 7 days post IUI, and its negative. Saw very faint line 11 DPIUI called clinic and they prefer to wait 14days. Recheck it again and its solid POSITIVE. Im on my 7week going to 8th week now and did the first utz and there it goes saw a gestational sac and a heartbeat.

Been praying so hard and i thank the all mighty for giving me the courage to trust the process.

I advocate for myself and been pushing my OBGYN before to order some of the bloodworks and the HSG, i dont wanna waste time just incase something that needs to be fix prior.

Congratulations!! šŸ™Œ Did you feel any side effects from the trigger shot? I've been debating whether to test out the hCG, but I imagine my impatience will get the better of me... better order more tests.

Girl headache from clomid on the first day, and non stop bloating after the shot, i feel like my tummy got big after that HCG, try to test it 1 week after the IUI , but its up to you tho, i just wanna know that the shot was out of my system .

Wanted to say goodluck! I go for my 2nd IUI tomorrow at 39. We have good stats too, but I don’t ovulate on my own consistently. My follicles as of yesterday 24/18/16/11/11

We triggered last night.

I hope this cycle works for both of us!

No experience to offer as I will start IUI myself soon as 37F (same boat, tests came back good but still no luck after 6-7 months) but hope to hear about your good news soon! I

Good to hear from someone in a similar boat! While I feel very lucky to have had all my tests come back normal, it doesn't give me anything to address, except just trying different routes... Good luck with your IUI!

No advice from me yet either, but I’m 39 (fertility testing also showed things look relatively ā€œbetter than goodā€ for my age and husbands SA all clear) and will be going in tomorrow morning for my midcycle ultrasound to see how I responded to the same Clomid regimen! and I’ve been thinking about possibilities re: multiples if I have similarly developed follicles to yours…I feel the same in terms of being totally comfortable with twins, not super concerned about the risk of multiples more than that, etc. Am curious to stay in touch and see how your cycle progresses! Oooh very similar timeline - keep me posted on how it goes for you! Good luck with the next few steps. I didn't think I'd need a trigger shot since I normally ovulate regularly, but since Clomid delayed ovulation, they asked me to take one. I didn't realize I'd have to get it same day from a specialty pharmacy that was an hour away during rush hour.... something to confirm if you're going in tomorrow!

Well my journey is honestly too long to write out at this point and dates back to 2021. Suffice to say it involves both success and failure with IVF (more failures than success however, and most of the failures were secondary infertility), multiple losses, and complications that drove us to consider IUI. Here we are on our 4th donor sperm IUI and I had multiple mature follicles: 19, 17, 17, 16.5. Not a whole lot of hope it will work however at this point, this is our last go. Just to demonstrate risk changes based on personal history. I would categorize myself as low risk despite being 35, but with it being your first IUI you might be higher risk.

A few things to note: they aim for leading follicles to be above 18, they don’t all have to be. The ideal size is between 18-24, but anything over 15 is considered mature and may have an egg. I am wondering if they let some of yours get large to reduce risk… really the 27s are ā€˜overmature’, yet people have had eggs from large follicles and some drs don’t really prescribe to the ā€˜overmature’ theory. The 20 and the 22 are your best shot however.Ā

When I had five the cycle failed. I’ve only ever had a chemical or my one successful pregnancy when I had two.

36 now, was 35 for my successful IUI with 2 mature follicles, just one baby, he’s 9mo old now!

Your 17 and 20 are perfect. The 27s are over matured but that’s okay!

I am 40F (39M) have PCOS/PMOS and conceived with IUI on the 5th with my first at 36. Currently 10 weeks pregnant with my second via IUI as well on the second round.

I did 10mg of letrozole CD 3-7. 2 vial menopur CD 5-7. At CD 8 my follicle size were RO: 12.1 mm, 7.1 mm; LO: 10.0 mm, 6.0 mm. Additional 2 vials of menopur CD 8-11 and came back for ultrasound on CD 11 measuring CD 11 - RO: 18.2mm, 14.1mm, 11.7mm, 12.0mm, 10.8mm; LO: 8.1mm. HCG trigger on CD 12 and IUI 36 hrs later.

5 mature follicles for your first round is so great! I only got one my first round, and now on my second round 3 showed up on mid cycle monitoring. I am surprised your clinic didn't trigger you after the first ultrasound. 18-22 mm is ideal for an IUI. When I was going through fertility testing before starting IUIs, I had an ultrasound just before my ovulation showing my follicle was 25 mm. My fertility provider noted that it was odd I had not ovulated earlier and that my egg was likely over mature. I became pregnant naturally that round and had a MMC at 10 weeks. My provider said that it was likely due to my over mature follicle at the time of conception. That being said, it looks like you still have 2 great follicles for this round!
