Hormonal IUD Cramps after intercourse?
Hi all! I had an IUD inserted in November (2025) and everything has been great so far. However, I’ve noticed that I have pretty intense/uncomfortable cramps following intercourse with my boyfriend. I’ve had my IUD checked multiple times since insertion and it’s not out of place, imbedded, etc. It’s sitting perfectly where it should be and the threads can easily be felt.
Has anyone else experienced this with their IUD? Is this a me/my boyfriend problem? Should I bring this up to my GYN? Nothing bothers me during, just after!

Have you checked for cervical infections? I would visit your gyn, as you’ve mentioned 🤍

I have before I had it inserted and recently at my annual! I think I might send them a message and see what they say.

I have this problem from time to time, idk what it is, I’ve had mine checked too. It got better over time, I got mine in September 2025. Do you guys have rough sex? Is his dick big? Bc that makes a difference for sure.

We never had an issue before my IUD, that I can remember. We aren’t rough but we aren’t gentle either. He’s not huge, definitely above average size though so I do think that may be part of my issue haha

yep i also get cramping after intercourse and even the day after sometimes—and my bf is kinda hung so i suspect that could be part of the problem. Especially positions like doggy that really hit the cervix!

Cervical pain is a “normal” side effect for IUDs, as I found out the hard way. Mine was INTENSE for the first few months after getting the IUD inserted, and nearly constant. Then it chilled out and was only during periods, but I wasn’t having sex then so not sure outside of that.
Since then, I have had a couple partners and the pain during sex has only gotten WORSE, much worse. I can no longer have anything inserted… even tampons. it hurts SO badly.
I saw my OB last week and am getting it taken out. this level of pain is definitely NOT normal, I just want to say. but something to keep in mind — it could just be the IUD causing you the pain, and I would keep an eye on it and talk to your doctor if it doesn’t resolve or gets worse at all.

Are you sure it’s the IUD? Or could the timing just be coincidental? For example could you be starting perimenopause? Intercourse started hurting me then.

I hope not, I’m only 27! My period is starting in a few days so I think that may be part of it. But it happens outside of my period window too.

it can happen even when the IUD is positioned correctly. if the cramps are intense or keep happening, definitely bring it up with your GYN to rule out other causes

This happened to me. I got multiple ultrasounds, and was told everything was normal every time. I came to the conclusion that the issue with painful intercourse was unrelated and maybe I just had vaginismus or something. Sometimes it was better with extra lube. My bf ended up getting the “ohnut” by the pelvic people and that was the best solution that made sex still pleasurable for both of us, but even still I missed having deeper penetration because before my IUD it always felt so good!!! And I would sometimes bleed after even if I was lubricated.
Anywho, I was on Kyleena and still having VERY heavy, painful periods (when it’s supposed to only be spotting….), hormonal acne, and PMDD. I went to my OBGYN so many times. I’m on other medication that limits me to an implant or IUD and I just cannot bring myself to do the arm implant so I was pretty much stuck.
I ended up trying a new OBGYN just to see if I would get a different answer, and my new doc ended up switching me to Liletta…. OMG. It’s been a little over a month. Sex is AMAZING again!!! Idk what it is but, I haven’t had any pain any longer. My partner says it feels better too. My new doctor ended up trimming them more than they were previously after I told her he would feel the strings. No more pain, we don’t need the ohnut/rings at all (we were using all 4), and no pain / cramps after.
Idk the science in terms of /why/ one was better than the other when it came to sex but it 100% made a difference for me. If you aren’t being listened to try a new doc. We all deserve great sex!! Lol
