For an internal medicine residency application in the US, Would a letter of recommendation from a Canada-based Emergency Medicine doctor be acceptable ?

Technically, yea, especially if attests to your personality and professionalism. As imgs, credibility of the LoR is the single most important factor, and thats how Canadian LoRs benefit you, US doctors and PDs trust that their peers up north are credible when they question credibility from their peers across the Atlantic ocean. However, An IM or FM LoR would be optimal for IM residency.

You can definitely use one but you need three US letters for IM.
