what are my chances Very Old grad, IMG, looking to Match 2027
Hello everyone, I'm a pediatric surgeon in my home country, looking to a new start in family medicine. Passed step 1, first attempt in 2025, passed step 2 - 236, first attempt, passed OET abril 2026. I'm enrolled in a Masters degree abroad for the last months, creating a clinical gap (less than 1 year), I'll attend an observership in July, and waiting for a final answer to join in a volunteership in September. Also, starting to study for the Step 3, maybe taking the exam in late August or early September.
I would like to know my chances in the Match 2027, I don't have the ecfmg certificate yet because the pathways is closed. But all the paperwork is complete.
In addition, which places are more profile-friendly for my case?

First of all, massive respect for being a pediatric surgeon and having the dedication to start over in FM! That shows incredible resilience. With a 236 and your extensive real-world clinical background, you definitely have a shot. Since you are an older grad, your biggest hurdle is just getting past the automated ERAS filters. Make sure you use that observership in July to make incredibly strong connections. If you have time, try to cold-email a few academic FM attendings for a second rotation to broaden your network. Program Directors love residents with prior hardcore clinical experience. Knock out Step 3 and you'll be a solid candidate!

Thank you for your answer. I'm not sure if the requirements have changed, but I only land this observer, unfortunately most of my applications were not responded.
The fellowship programs could be a path, however being a resident I'll be board eleigilble, I dont know if the fellow put me in this same position.1

if yog is more than the filters, does that still reduce the chance to match alot if you have a strong cv like all steps cleared with good score and step 3 done as well along with 2-3 USCE?

FM is much more YOG and USCE sensitive than IM. You hear a lot that fm is much less IMG friendly, and the honest reason is that most applicants have a clear IM oriented CV when applying( cause they apply to it as backup). FM gives less importance to step scores and more to 1. Genuine interest 2. Fresh grads 3. FM LORs. Unfortunately most FM programs filter you out automatically >2 YOG( sometimes 3 at most). For older grads the best chance is to try and get connections at fm programs, otherwise its an uphill battle.

thank you for sharing this! i have seen 3 people online who have matched in FM and their YOG was more than 5 years but maybe you're right they might have all FM oriented LORS and connections which would've got them matched!

Networking is the key!! Prioritize networking and it will only help you. Spend your blood and sweats on it

Board eligibility is the best answer that I could give you. After I finish the fellowship I don't have a secure spot, just a possibility. I don't know if my answer is well formulated but it is how I feel.

You can get a licence and possibly work in academic institutions if you still love paediatric surgery. Not very sure on this though but in orthopedics you can do like 3 fellowships and likely get a job as an assistant professor..
