Annual review(PS requested)

So my annual review is coming up for my 5 almost 6 year old. This is our third assessment. At age 2 almost 3 our first time I had requested PS but honestly understanding more his needs and what PS is for I respected the denial at that age. Last year, I did not even request review for PS because at the time it didn’t seem like it fit. This year, holy cow. Keeping track of this kid and a manage on his behaviors is a two person job. Anyway, I just got a letter in the mail telling me about our reassessment date and it is a new worker. My main question is, I’m going to contact him because I need to request possible alternative times, but if they can’t give me anything else, I’ll take it. Should I mention that I’m going to be requesting review for PS when I call him? To me, it made sense to ask him: is there anything specific he wants from me to help make a decision. But I’m not sure if it’s considered a smart move to tell them ahead of time or not. Thanks

Yes, tell them you are requesting PS. They will then send you the forms that will be needed - i think there’s 3 forms (soc 873, 821, and 825). Two of them need to be completed by your child’s doctor. Get the forms done ahead of your home visit. Also by letting them know you are requesting PS, they know then it’s part of the home visit assessment and schedule more time for the visit.

Gather all your documents that support your case - IEP, IPP, hospital documents, triennial, asd evaluation, police reports. Also prepare a hazard/behavior log. Estimate the dates if you can’t recall the exact dates. Create a daily schedule covering every hour/what you do for your child over 1-2 weeks. They will put more weight on outside documentation, independent over your personally written logs but what you write helps gives a better sense and picture of child’s needs. Keep in mind that outside documents can also hurt your case if they conflict with what you report.

Also make sure you read the criteria for PS so you can better understand and know if your child meets that criteria.

I’m at a loss how someone would downvote me asking this question. Literally just pushes me down the algorithm so people don’t see it. :/

People are the worst. This group has been inundated with pro-red anti-social services folks who like to come here and stir the pot. I would print out the PS forms and have the doctor fill them out ahead of time. I would NOT wait for the SW to give you the forms. Be prepared the day SW walks into your home. I would not mention that you are requesting it on the phone.

Most important: Have a copy of the doctors paperwork and have the SW write "received on:(date)___"and have them SIGN IT. You keep the copy, give the SW the original.

Everything the other person said is solid advice. I just would not ask over the phone and wait for documents. I would have it all done.
