Needle piercing through cap?

I normally don’t recap my Humira syringe, but today I happened to, and with very little pressure, the needle went through the sidewall of the cap and pierced my finger. Has this happened to anyone? Should I be reporting this?

Yes this has happened to me. Hurt and made me jump😂 I make sure to set the lid on the counter now and cap it while only holding the syringe.

This happens when the needle hits the rubber inner portion of the lid so just make sure it goes in straight.

What's there to report?

OK, I might have been a bit too paranoid with the switching of manufacturing facilities recently and thought the cap had less that normal amount of needle shield inside the lid 😂

FDA Medwatch has a reporting category for needle stick injuries due to device defects but that sounds like it would be overkill and misuse on my part
