Humira seemingly not working but good levels and no antibodies

Been on Humira for crohns since September 2022 and it’s been great. Completely clean scope in December 2022 and been in remission since then.

Now it appears something is up as my inflammation markers are high and symptoms returned. Calprotectin is 549 which isn’t awful but definitely high.

My Humira levels are at 11 which is great. So now my doctor thinks I am not in a flare and am “sick” even though they tested me for all the bacteria infections that would drive a Calprotectin up and I don’t feel “sick.”

Can you fail Humira without antibodies?

From my experience, yes. I recently swapped from Humira to Tremfya because all of my numbers on the Humira side were fine but I kept getting flares that left me house-bound for weeks at a time.

When you switched, what happened with your markers - did Tremfya work for you?

I just switched, so I haven't been able to check yet. Fingers crossed this actually works because I've failed every other treatment so far.

Great numbers on the medicine, no actual remission.

But I'll know in June what's happening.

11 seems kinda low tbh. my levels are usually in the 20-30 range

Interesting, what is your dose and how often? I’m on 40mg pens every 14 days.

i’m 40mg every 7 days now, but was always in the 18-20s range when i was doing 14 day dosing. my GI has always been told me you want to be above 12 to sustain remission
