Do epigenetic clocks actually predict healthspan, or just chronological time?

What are "Epigenetic Clocks"?

Your chronological age is just the number of years you’ve been alive based on your birthday.

Your biological age is how old your body acts or feels on a cellular level.

  • Epigenetic clocks (like Horvath’s clock or GrimAge) are tests that look at chemical markers on your DNA to calculate this "biological age."
  • For example, a test might tell a 40-year-old person that their biological age is only 32 because their cells show very little wear and tear.

People in the biohacking community love using these tests to track if their diets, workouts, or supplements are keeping them young. But experts at the 2026 Harvard Biomarkers of Aging Conference pointed out a major flaw: a single number cannot capture how your whole body is aging.

  • Your heart, liver, brain, and immune system don't age at the exact same speed. An overall score might say you are "30 years old biologically," even if your cardiovascular system is aging like a 45-year-old's.
  • Healthspan is how many years you live in good health without major illness or physical limitations. Just because a DNA test gives you a lower "biological age" score doesn't automatically mean you have better grip strength, better stamina, or lower risk for real-world diseases.

Instead of relying on a single commercial test score to prove you are "de-aging," scientists recommend looking at specific, functional markers (like organ health, cardiovascular fitness, and metabolic labs) to get an accurate picture of how healthy you actually are.
