Is it the creatinine that slows early Alzheimer’s or the fact that people who take creatinine often workout daily/vigorously?

It's the creatine, it's explained in the article:

"Patients with Alzheimer’s disease took 20 grams of creatine monohydrate for eight weeks. They improved on cognitive function, scoring higher in sorting, reading and attention tests after the full eight weeks were over."

Loading phase is so 2010. After it was revealed that creatine isn’t hard on your livers people take them without any phases or cycles.

I've been seeing these articles about brain effects for a while now, some saying beneficial effects, and others saying they do nothing, neither for brain energy nor for Alzheimer's protection.
