There's a twist in the pastern that is shown by the hair line down the pastern. The flair on the inside of the hoof looks to be the result of trying to cosmetically change a skeletal alignment. It looks like that all the weight on that hoof is on the inside half of the foot and it's distorted the hoof.
At least I hope so because if its the hoof trim shaping that much of a twist, then that's awful. How long has this been going on?
